On your first day, you complete a full assessment. Clinicians review your history, physical health, mental health, and substance use. That information shapes a treatment plan built around your specific needs, not a template.
You also get a dedicated case manager. This person tracks your progress, connects the pieces of your care, and plans your step down into aftercare. When PHP ends, you already know what comes next.
